NURS4704: Care Transitions Across the Continuum of Care

This course focuses on care transitions across the continuum of care and the care coordination role of the nurse to support optimal outcomes for patients and families. Strategies for care transitions and coordination will be explored and students will have on-site practicum experiences in care settings with a focus on care coordination and care transitions.
